To obtain an apostille from the Office of the Lieutenant Governor Salt lake City please review the following Considerations to make sure your documents will be acceptable:

Guidelines for Birth and Death Certificates must be issued by Vital Records Dept. and signed by the State RegistrarJanice L Houston or Barry E Nangle.

Guidelines for Marriage License and Single Status Letter must be issued from the County Clerk Offices where the marriage took place.

Guidelines for Divorce decrees must be issued by the court and signed by the Judge or Court Clerk

Guidelines for Certificates of free sale must be issued by the Utah Department of Agriculture

Guidelines for Police Clearance documents should be issued and notarized the Police department.

Guidelines for Business documents must be notarized at the Dept. of Commerce. Please note that certified copies alone are not acceptable for Apostille.
